REGISTER WITH THE PRACTICE?
If you live within the practice area, you can register by coming to the practice and handing in a completed NHS medical card. If you do not have a card, but are eligible to receive NHS care, you can still register by
completing a registration form at the surgery. If you are not sure if you are eligible for NHS care, or are unsure if you live within the practice area, please ask the receptionist.
Please note that although we have some patients who live in Southowram and Boothtown,
we no longer accept new patients living in these areas, as they are now outside
our practice area.
When you register you will be offered an appointment with the practice nurse for
a new patient health check. We strongly advise you to attend this appointment,
especially if you are on medication or have not been to see a doctor for some
time.
If your personal details or address changes, please inform the practice as soon
as possible so that we can amend our records. Unfortunately, if you move outside
our practice area, we will no longer be able to keep you as a patient.
