OBTAIN A REPEAT PRESCRIPTION?
Patients on repeat prescriptions will be automatically issued with a white re-order slip whenever they obtain a prescription. This should be torn off and used to order a new prescription by ticking the box next to the
item that is required. If you do not have this slip, please put your request in writing as we do not accept requests over the phone, except in an emergency.
Please allow 48 hours from the time of your request before coming to collect your prescription. If you need the prescription posting to you, please enclose an SAE with your re-order slip. You can also arrange for
the local pharmacist to collect and dispense your prescriptions for you.
At the bottom of your re-order slip is a review date. If your review date is imminent
or has passed, you will be asked to make an appointment with a doctor or nurse
for a check-up before your next prescription.
Please note repeat prescriptions for the oral contraceptive will not be issued
without seeing a nurse for a check-up. Please make sure you make an appointment
before you run out.
